5. Northwell Health Stern Family Center for Rehabilitation
Northwell Health Stern Family Center for Rehabilitation is a nursing home based in Manhasset, New York. It is one of the larger centers on our list, with a total of 256 beds. The center is governed by a resident council and it is a corporate and nonprofit entity. The nursing center does not provide continuing care, but it provides short term rehabilitation and long term care.
4. Life Care Center – Longmont
Life Care Center is a nursing center in Longmont, Colorado. It has 187 beds and is a for-profit corporate entity that is governed by a resident council. The nursing home does not offer care for Alzheimer’s disease, nor does it offer continuing care. However, short term and long term care are offered. Other services that the Life Care Center offers include post surgical care, IV therapy, discharge planning, and medication management. Its services also include physical, occupational, and speech therapy.
3. Hebrew Home of Greater Washington
Hebrew Home of Greater Washington is the biggest nursing home on our list. It has a whopping 558 beds in its facility. The nursing home offers Medicaid and Medicare coverage and also long and short term care. While the nursing home does not offer Alzheimer’s support, it does offer memory care, which includes housekeeping and dementia care. It also has assisted and independent living options, with cognitive therapy and other services as well as an emergency response system.
2. Hunterdon Care Center
Hunterdon Care Center is based in Flemington, New Jersey. It has 185 beds and is a for profit organization and a limited liability company. 69.1% of its residents were able to return home after being discharged, higher than New Jersey’s 55.4% average and the 53.8% national average. Its rehospitalization, fall, and infection rates are also below the national average. Additionally, Hunterdon Care Center is also a rare facility on our list that also offers Alzheimer’s care which includes cognitive programs and operates as a separate, functional unit.

Burgess Square is a nursing home that is located in Westmont, Illinois. It has 203 beds and is a for-profit, partnership. The nursing home does not provide Alzheimer’s care. Its programs and services include physical rehabilitation, wound care, cardiac care, pulmonary care, and palliative care. The physical care services include recoveries from surgeries, joint replacement, and Parkinson’s Disease care among others. Its heart care segment cares for patients with heart valve replacement, heart transplants, and angioplasty.
